Search for clinical and neurophysiological factors associated with the stability of therapeutic remission in alcohol use disorders.

Aim. Identification of clinical and neurophysiological factors associated with the stability of remission in patients with alcohol dependence after previous inpatient treatment. Methods. 170 patients with alcohol dependence were examined. The following scales were used as the main psychometric tools: the Hamilton Questionnaire for assessing the severity of anxiety (HARS), the Snaith–Hamilton anhedonia scale (SHAPS), the Dependence Severity Index (ASI), the obsessive-compulsive scale of attraction to alcohol (OCS), the Pennsylvania Scale of Attraction to alcohol (PACS). The background bioelectric activity of the brain was recorded, the values of spectral power and coherence of δ- (0.5–3 Hz), θ- (4–7 Hz), α- (8–13 Hz) and β- (14–30 Hz) rhythms were analyzed. Results. According to the data of clinical and dynamic indicators, a significant factor associated with the duration of remission is the level of anxiety on the Hamilton scale (p=0.006). EEG parameters (in the amount of 9 pieces) were also identified, which had the most significant effect on predicting the duration of alcoholic remission in patients: the spectral power of the alpha rhythm in the F4 lead, the coherence of the theta rhythm in C3–C4, the coherence of the theta rhythm in C4–O2, the coherence of the theta rhythm in F7–F8, the coherence of the theta rhythm in Fp1–T3, the coherence of the theta rhythm in T4–O2, the coherence of the alpha rhythm in Fp1–T3, the coherence of the alpha rhythm in P3–P4 and the coherence of the beta rhythm in Fp2–C4. Based on the data obtained, the equations of discriminant functions were constructed. The sensitivity of the developed model was – 84.4%, specificity – 86.5%. Conclusion. The study showed that there are significant differences in the level of anxiety and brain functioning in patients with alcoholism, depending on the duration of remission.
