Is an impaired emotion recognition a predictor or a consequence of alcohol abuse? Comparison of data from healthy participants and patients with borderline personality disorder.

Introduction. The fundamental question of whether a violation of emotion recognition is one of the causes of addiction or is a consequence of the disease remains open until now. Aim: to study the features of emotion recognition in people with alcohol consumption, as well as in patients with borderline personality disorder, in comparison with healthy individuals. Methods. Emotion recognition was assessed using the Emotion Recognition program based on the Karolinska Institute’s Emotional Fascial Expression database (KDEF-dyn data base). We measured the percentage of correct and incorrect answers, as well as the average emotion recognition time (ms). The experimental procedure consisted of 240 video clips depicting basic human emotions: happiness, sadness, anger, fear, disgust, surprise. The study involved 108 patients, of whom 31 were single drinkers (without psychopathology), 32 patients diagnosed with Borderline Personality Disorder (BPD) and 45 healthy participants. Results. The results show that BPD patients and participants with alcohol use not only have sadness recognition impairment, but also common patterns of emotion misrecognition: misidentification of disgust instead of sadness. Conclusion. It is possible that the role of impairment in the recognition of sadness underlies the formation of alcohol consumption (and possibly addiction) and exists along with other psychopathological and individual psychological characteristics. It is quite possible that impaired recognition of emotions by facial expression is a marker of alcohol consumption.
