Polymorphism A118G (rs1799971) in Ц opiod receptor (OPRM1) gene is considered as afunctional one: G allele encodes an amino acid substitution in the structure of ц-opiod receptor, which leads to a 3-fold increase in affinity for the beta-endorphin and can affect the level of genetic risk for subs-tance abuse and the effectiveness of treatment of substance dependence (SD) with opioid receptor antagonists. Objective: Comparative analysis of association A118G polymorphism with alcohol and opiate dependence with quantitative measures of family history on SD in inpatients, men, ethnic Russian: alcohol dependence (n = 346), dependence on opiates (heroin) (n = 194) and in control group of healthy individuals, men (n = 143). A118G alleles and genotypes frequencies do not differ in the groups of patients and control group, including the analysis of the degree of family history on SD and A118G polymorphism is not associated with risk for alcohol or opiate dependence and family history on SD. Further research of gene polymorp¬hism of endogenous opioid system is needed, involving clinical, psychometric and psychopathological variables. The combination of individualized pharmacogenetic approach and analysis of indirect effects A118G polymorphism in conjunction with the assessment of different dosage forms of opioid receptor antagonists will reveal the conditions of maximum effectiveness of these drugs in selected groups of patients.
